After surviving the AIDS epidemic of the 80‘s gays and bisexual men are now fighting another life and death battle.While it may have come as a surprise to politicians, anyone reading the overseas gay press knew it was on the way.Paul Schulte is an experienced drug and alcohol counsellor. He looks at the reasons why this drug is so attractive, and how the addiction can be treated once it takes hold.First he provides the background: why there's such a high incidence of substance abuse in the gay community (3-4 times the general population, and even higher for bisexual men). He points out that a high percentage of gay and bi men suffer from bipolar disorder. Many were victims of child abuse. And a great number of the now middle-aged suffer from something like PTSD, after seeing so many of their friends and lovers die of AIDS in the 80’s and 90’s.So, you take anything you can that makes you feel better.As far as meth is concerned: you enjoy great sex... the best ever (for a while); you're full of energy; worries about contracting or having HIV magically disappear; and all the guilt and shame accumulated over years, vanishes. Sounds wonderful.But it’s a drug that has a terrible down side; not only is it highly addictive but, for many users it produces psychosis, paranoia and violent behaviour. It’s also led to a dramatic rise in HIV infection.In short, it's bad news.In Schulte's experience the only thing that’ll get you off meth is a combination of therapy, medication and participation in 12-step groups. That, and volunteering to help others in the same situation. This is the necessary Twelfth Step in A.A., the passing on to others what you're learning yourself. “Nothing brings about more healing than working to heal that which bothers us most."Because organized religion has failed most gay people, drugs like meth are used to fill the gap and kill the pain. Through the insights of psychologists Viktor Frankl and William James, Shulte explains how suffering (shame, rejection, fear, addiction) can actually bring about an existential crisis (what in A.A. circles is referred to as hitting bottom) that is the essential first step to recovery. Meaning and direction (and continued sobriety) are then found in the company fellow recoverers because they know what you're going through, having been there themselves.The PTSD suffered by gay and bisexual men is similar to the trauma suffered by combat veterans of the Vietnam and Iraq wars. In both cases drugs and alcohol have been used to self medicate. He draws a striking analogy between the darker side of gay and bisexual experience and Post Traumatic Stress Disorder. His conclusions - the need for a franker and more honest debate, and for treatment that combine medical, psychological and spiritual approaches, are clear and convincing - and timely, given the scale of the CMA challenge in North America, Europe and increasingly in Asia.
