Felitti, MDInscribed BodiesAnna Luise Kirkingen MD, PhDKluwer Academic Publishers, 2001Book explores three aspects of human life:* Integrity violation* Effect of sexual violation on human health* Biomedical intervention into illness produced by sexual violationSexual abuse is not recognized because it is in the nature of biomedical practice and its attempts at `objectivity' to obstruct the disclosure of social pathology. When treatment fails in this system, either the knowledge base is re-evaluated or it is not. The latter choice is overwhelmingly made by the non-recognition of sexual abuse because the necessary knowledge lies outside biomedicine, and is in interpersonal relationships and transactions. This realm readily is dismissed as anecdotal and hence unscientific; they are also threatening subjects concealed by shame, secrecy, and social taboo. This situation is amplified by many religions that treat sexuality as a non-topic, and by the fact that in every society there are certain violations so stigmatizing that the victim does her or his utmost to prevent them being known.Humiliation and degradation lie at the core of the damage produced by sexual violation. These lie outside the biomedical model, as does a coherent explanation for the symptoms of somatization disorders. In the biomedical model, a distinct and unitary cause is sought, not causality resulting from a complex context. The intellectual threat this alternative poses to physicians is significant because it challenges the very basis of their professional being, in addition to often reviving the memory of events within their own life experiences. To the degree that risk factors are acknowledged, they are attributed to personal behavior choices, not to interpersonal relationships. Patients subtly perceive this, and then become even less likely to mention spontaneously their adverse life experiences. The silencing of victims is the core phenomenon of abuse, a prerequisite for its occurrence. Medical practice thus colludes with society. Even intended kindness can mean inadvertent violence if its effect is similar to that of violence - the `loving offender'.As a result of this inability to speak openly, somatization disorders develop, thus becoming an adaptive strategy for seeking help, but in disguise. Hidden and forbidden experiences become integrated into lives and bodies in this manner; when powerful feelings are not acted out they are acted in. The telling of experience is needed to confirm meaning; without this, powerful feelings are likely to be forced into one's unconscious; in which case the body will remember what the head can not. Psychosis is the breakdown of one's adaptation to silenced trauma; adaptation becomes maladaptive. This is analogous to the ACE Study findings about addiction. Moreover, the effect can be used to discredit the reality or validity of the cause: How can one trust what an addict saysThe impact of abuse can be accounted for by four dynamics: stigmatization, betrayal, powerlessness, and traumatic sexualization. The destructive potential of powerlessness is enormous; the random infliction of pain and humiliation is the essence of torture. Betrayal, the unmaking of trust, affects significant relationships, life-course, sexuality, and individual health.Disbelief in the authenticity of abuse memories results in the collective silencing of those voices that speak out about such violations. Lack of confirmation by mothers of these memories is singularly destructive. Linda Williams (J Traumatic Stress 1995; 8:649-73) carried out an important study showing that about 40% of confirmed cases of childhood sexual abuse are not cognitively recalled by those individuals as adults. This brings us back to the conflict between disease as biomedical pathology and illness as experience.Pandora's Box is the metaphor for that which is overwhelming, that for which one has no coping skills. If one is overwhelmed by time demands, what would happen if time were not there as the explanation Would there be a hidden, far more threatening explanationIn all of this, it is sad that medical interventions commonly maintain if not create sickness. In this sense, sickness is a breakdown of adaptability to chaos. Inadequate diagnosis leading to treatments suggestive of violation are reminiscent of the basic problem in which adults abdicate their responsibility but not their control. Many of the diagnoses are actually descriptive of coping mechanisms, which then are treated in the sense of removal of the obvious coping strategy: social phobia, addiction, etc.There are 4 central elements in children's experience of abuse:* Traumatic sexualization* Betrayal* Powerlessness* StigmatizationCommon defense strategies in response to the above are:* Denial* Repression* DissociationUnfortunately, though comforting, these operate so as to increase the likelihood of revictimization. They also increase greatly the likelihood of somatization; this emphasizes the need for early diagnosis of somatization and treating it properly by seeking its origins. This also reduces the length of time a patient is exposed to the stigmatization resulting from repeated proof of non-pathology in the medical sense, or traumatizing diagnostic procedures. There is a double trauma here: that of not confirming a hidden reality, and that of applying procedures and treatments that are themselves traumatic.
